The Clerk’s Office provides many services to residents and visitors of Worth Township including: voter registration, temporary handicap parking permits, notary public services, passport acceptance and US travel brochures. Monday through Thursday: 7:00 am to 3:30 pm
Friday: Employees work remotely, please call or email.
Passports are by appointment only
PASSPORT ACCEPTANCE
Our Clerk’s Office has acceptance agents to assist you in obtaining a passport book or passport card. Appointments are required. Applications are processed Monday – Thursday from 7:00 a.m. to 3:00 p.m.
Current passport processing times are 3-4 weeks for expedited service and 6-8 weeks for routine service.
Mailing times are not included in processing times. Consider the total time it will take to receive your
passport when you are booking travel. (It may take two weeks for your application to get to them, and two weeks
for you to get your passport after they mail it).
When applying for a passport book or card you must have the following:
The completed, unsigned DS-11 application (applications available at our facility)
Proof of US Citizenship
Example – Previous passport, certified birth certificate, original naturalization papers
*You must provide your Social Security Number on form
One passport picture (2×2 inches)
We DO take pictures at our facility for a $15 fee.
Proof of Identity
Example -Drivers license, State I.D.

Information can be requested of the Township of Worth under the Freedom of Information Act (FOIA) (5 ILCS 140/) in writing. Please contact us via email at elwoodk@worthtownship.com (please put FOIA in the subject line), mail, personal delivery or fax.
